What's it got to do with you leafletWe are committed to providing excellent services that are responsive to your needs. In order to do this it is essential we hold accurate information about you and any specific needs you may have. We will from time to time ask you to share and confirm information with us about your;

  • Ethnicity
  • Religion
  • Language Needs
  • How you would like us to contact you?
  • Disability
  • Sexuality

You don’t have to give us this information, but please tell us if you would prefer not to.

If your circumstances change it is really important that you let us know as soon as possible by contacting us.

If you are a customer and you haven’t already completed a customer profile form, we would be grateful if you could spend a few minutes to do so. Click on the link below to download the form if you need help please telephone us on 0191 385 1900 or call into the Cestria Hub.

If you require any assistance filling in the form please let us know and a member of staff will come out and visit you.

"Getting to know you" information

When we ask you to complete a questionnaire, we'll also ask you to give us information about yourself (demographic information). We understand that sometimes people find it difficult to understand the relevance of collecting information and what this is used for, especially with regards to sexual orientation and religion or belief.
